Use these forms to make requests and offer feedback to the Library at Southwest.

We welcome your feedback about our performance. Share your thoughts on our customer service, resources we have for your use, services provided, how we can improve and what we are doing right.

Customer Survey

We can help you search for and borrow items Southwest does not own from other libraries that take part in this program. Please allow up to two weeks to fill a request. Provide as much information as possible.

Inter-Library Loan Periodical/Book Request

Librarians provide a 45–50 minute hands-on session on the proper use of library resources and services. Sessions can occur in a computer lab, classroom with computers or online (webinars). We provide both general orientations and sessions tailored for a specific assignment, like career research, research papers or public speaking.

Library Instruction

We want your feedback to help improve future presentations.

We welcome your suggestions about which resources we should purchase to enhance our collection. The library executive director makes the final decision on these purchases. Provide as much information as possible.

Material Request

Instructors may place textbooks and other items on reserve for students to use on-site in the library for two hours at a time. The completed form should accompany the resources. The library does not purchase books for this collection. 

Material Reserve

Students may complete this form once a semester to check out mobile devices, such as tablets and iPads. These items may be used on-site in the library for three hours at a time. Late fines apply.

Use this form to renew checked out materials or log in and renew them in the online catalog. 

Online Renewal 

Students can request copies of a limited number of pages from a reserve textbook. We will send scanned pages to the library of your choice, and you can copy the pages for .5 cents per page. Requests are processed within 24 hours. Expect to receive emails about the status of your request. You cannot use this service as a substitute for purchasing a textbook.
