Certifications and Signatures

This form confirms that all individuals signing certify the information provided on the FAFSA and related documents is accurate and complete.
Dependency Override Appeal

Students who cannot provide parental information due to unusual or extenuating circumstances may request a dependency override. Financial Aid administrators may use professional judgment to review documentation and determine eligibility.
Financial Aid Authorization Form

This form allows Southwest to apply your financial aid funds toward tuition, fees, bookstore charges, and other allowable institutional expenses. Income 
Income Tax Information (Student or Parent) - Get Transcript

Tax Information – IRS Tax Return Transcript (Student or Parent) If selected for verification, you must submit the required IRS Tax Return Transcript for the tax year indicated in your My.Southwest account.
Orphan/Ward Of The Court/Foster Care Form

This form verifies your independent status based on your responses to the dependency questions on the FAFSA. Please note that additional documentation is required to support your request for independent status.
Professional Judgment Appeal

Students experiencing significant changes in financial circumstances may request a professional judgment review. Documentation is required to support the appeal.
Unaccompanied Homeless Youth Form  
Unusual Enrollment History Form This form is required if your enrollment pattern shows attendance at multiple institutions over the past four academic years. It helps determine continued eligibility for federal aid.
